Other services available as private pay.  Upon request, we will provide a superbill to allow you to self-submit to your insurance for out-of-network reimbursement. May require MD referral if submitting for insurance reimbursement. Please contact your insurance company to verify your out-of-network benefits PRIOR to receiving services.

In accordance with the No Surprises Act, private pay patients have the right to receive a "Good Faith Estimate" (GFE) explaining how much your medical care will cost This will be provided prior to the start of your services. Please request your GFE today or contact us to discuss the cost of services.
